Listed below are web forms available through the AHC-IS:

Data/File Server Access Request Form
The Data/File Server Access request form is used to add/modify/delete server access for an individual. This form should be submitted whenever someone joins or leaves your department, or when someone needs modification to their current server access.

Meeting Maker Request Form
This form is used to add or delete Meeting Maker accounts for people, resources, or locations.

Fairview Clinical Applications Request Form
This form is used to request the addition or removal of access, or installation of Fairview applications such as PAC’s, FCIS, or Fairview VPN.

Collaborative Account Request Form
This form is used to request a new Collaborative Account or to request modificaitons to an existing account. A Collaborative account is a single Active Directory account ID/password used by multiple individuals to obtain access to an AHC-IS supported workstation or server, and it will remain the discretion of AHC-IS whether a Collaborative account is appropriate for each particular request.

Add Existing Computer to Support Form
AHC-wide policy states that all computer purchases should be made directly through AHC-IS (via the Request for Purchase form). However, in the case of an exception, the new Add Existing Computer to Support form is used to request that an existing University-owned computer be added to AHC-IS support.

Request for Purchase Form
The new Request for Purchase form is used to request the purchase of computers, hardware, or software with University funds. This form features three purchase options: Standard Build Computer, Custom Order Computer, and Other Peripherals.

AHC-IS Server Add/Change Request - This form is for requested new servers, changes to existing servers and new hosted servers.

Tips on submitting AHC-IS request for service forms:   

  • When an employee joins your department, submit a Data/File Server Access request form, selecting the “Add Access For This User” option at the top of the form. Additionally, if the new user needs any Fairview or MeetingMaker access, submit each of those forms separately. If you need to purchase a computer for the new employee, submit a Request for Purchase form, or submit the Add Existing Computer to Support form to add an existing computer to AHC-IS support.
  • When an employee leaves your department, submit a Data/File Server Access request form and select the “Remove This User” option at the top of the form. AHC-IS will then remove any server access s/he may have had and will also inactivate the user in the AHC-IS internal database. If the employee had any Fairview or MeetingMaker access, submit each of those forms separately and select the “remove” option to remove that access.
  • Before you fill out a web form, take a look at the entire form for any required information you may need to gather first. Have all necessary information such as X.500, AHC-IS device ID, EFS string, etc., ready before filling out the form. For Fairview requests, be sure to have information such as Caregiver ID, last 4 digits of SSN, and printer info ready if necessary.
  • In the Department drop down list on each form, look for your department listed alphabetically. If you do not see it, look for it listed under the parent college/area (i.e.: Medicine, Medical School, Health Sciences, etc).
